Experience
Since 1999, SCC has been providing professional Geriatric Care Management (GCM) services including assessment, care coordination, care planning, monitoring, and crisis intervention.

Responsibility
Our professional staff stays up to date on the status of the client and makes changes to the care plan as needed.  Contact with family members is maintained on a regular basis.  We take seriously the ongoing responsibility to promote the health and well-being of the client.

Encouragement
We promote the autonomy of the elderly person, inviting their participation in the decision-making process and encouraging them to regain and/or develop their capacity to the extent possible.  We always use the least intrusive methods to achieve these goals.
