What is Care Management?
Geriatric Care Management (GCM) helps older adults and their families develop a care plan that fits.  The Care Manager assists older adults in reaching their full active potential.  Additionally, the Care Manager acts as an experienced guide and resource for families of elders.

What does a Care Manager do?
A Care Manager conducts in-home assessments, arranges in-home care, and coordinates other services.  Care Managers are also involved in crisis intervention, counseling, assistance with moving, and referrals to other supportive services.  The Care Manager acts as a liaison to families, overseeing care and quickly alerting them when there is a problem.
